Student Agreement and Policy Acknowledgment

By registering for a CPR or BLS class with Vital Care Services Inc., I acknowledge and agree to the following:

1. I understand that I must pay for my class before scheduling a date and time.

2. After payment, I must click the “Book Now” tab to schedule my class.

3. If I would like a hybrid class option, I will indicate this in the notes section when registering.

4. I understand that all sales are final and there are no refunds.

5. I understand that if I do not show up for my scheduled class, I will not receive a refund.

6. I understand that I must provide at least 24 hours’ notice if I need to cancel or reschedule my class.

7. I understand that if I do not provide at least 24 hours’ notice, I will be charged a $50.00 rescheduling fee.

By registering for this class, I acknowledge that I have read and agree to these policies.
